Sign-in Procedures

On arrival,

  • Gold members - should show their membership card to the membership representative.
  • Bronze members - should show their membership card and present one training voucher.
  • Non-members - should present one training voucher.[NOTE AT PRESENT WE HAVE SUSPENDED NON-MEMBERS FROM TRAINING WITH THE CLUB FOR INSURANCE REASONS]

NOTE: If you cannot complete the relevant step above you will not be able to take part in the session.

All participants should complete one row of the sign in sheet:

  • Name - Your first name and surname
  • Membership Type - What type of MTC membership do you have - Gold, Junior Gold, Bronze, Junior Bronze, Non-member?
  • Medical Conditions? - Do you have any medical conditions that may affect your participation in the session or that the coach should be aware of? - Answer YES or NO.
  • Special Requirements? - Is there any other information the coach should be aware of that may affect your participation in the session? - Answer YES or NO.
  • Signature - Sign to confirm that the information is correct and that you have read and understand the following disclaimer:

Disclaimer: By signing this sheet you are declaring yourself fit to take part in this session. If you have ticked the medical conditions or special requirements boxes it is your responsibility to highlight this to the coach/s and complete and sign an MTC Participant Information Sheet. This sheet is only valid for that session and a new sheet will need to be completed at subsequent sessions.

If required complete a Participant Information Form:

  • If you have answered YES to having any medical conditions or special requirments that may affect your participation in the session, you must complete section 1 of the Participant Information Form: Medical Information
  • If you are a Non-member then you must complete section 3 of the Participant Information Form: Emergency Contact Details

NOTE: This form can be downloaded from the MTC website. If you know you will need to complete one why not download/complete it in advance and hand it in when you sign-in.

  • If you noted in section 1 of the Participant Information Form: Medical Information, that you required access to certain medications during the session then place it in a see-through plastic bag labeled with your name and date of birth and place it in the Medication Box. This way should a situation arise where you required access to the medication both you, the coach and the emergency services will know where to locate it.

NOTE: This process must be followed at the start of EVERY SESSION. The coach will store any Participant Information sheets for his/her records, but they will not be available at future sessions. If you have a long term medical issue or special requirement then speak with a member of the coaching team about purchasing a wristband. Alternatively complete the participant information form in advance of the session.


NOTES FOR COACHES

Sign-In sheets now comes in the form of books.  There are now 2 sections to each sheet in the book. There is a main tear-off sheet for you where attendees can sign-in and a stub for some other information that is required by the club. IT IS YOUR RESPONSIBILITY TO MAKE SURE THAT BOTH SECTIONS ARE COMPLETED.  YOU CAN TAKE AWAY THE TEAR-OFF SLIP BUT THE STUB IS REQUIRED BY THE CLUB.  There is a separate book for each session at each venue.  So for example there is one book for the Sat AM swim session at Moss Side and another for the Tues PM swim session, etc. The books are stored in the coaching box/s at each venue.

As a coach you must complete the following sections of the Sign-In sheet before participants arrive:

  • Venue (Moss Side, Aquatics, Stretford, Longford), Session (Swim, Bike, Run), Date, Time - This should be done on the tear-off section AND the stub.
  • Human Resources - Coach 1, Coach 2 and collector
  • Health & Safety - Risk Assessment Reviewed? Risk Assessment Implemented? Safety Briefing Delivered? Equipment Checked?
  • Session Plan - Include an outline of the session you will deliver

Check the sign-in sheet at the beginning of the session:

  • Have all non-members, people with a relevant medical condition or people with a special requirement identified themselves to you and complete a participant information form?
  • Do the number of people on the sign-in sheet matches the number of people participating in the session?
  • If the answer is no to either of the questions above try to identify the missing individuals and ask them to leave the session.
  • If they cannot be identified then stop the session and ask them to come forward.

You must complete the following sections of the Sign-In form at the end of the sessions:

  • Injuries and Accidents -
    • If any occurred make a brief note here and the location/ref no of the relevant accident form
    • If none occurred make sure you write ‘NONE' in this field.
  • Attendance information summary:
    • Number of participants
    • Number of bronze
    • Number of gold

Following the session you should remove the tear-off section of the Sign-in sheet using the perforations.  Store the sign-in sheet and any completed participant information forms for your records. This information is useful to the club for auditing purposes and essential for your protection should an issue arise in the future. Make sure the book is returned to the coaching box safely with the slip fully completed.

At the end of each quarter the lead coach at each session is responsible for the safe return of the completed Sign-in book for their session to the Head Coach.  The dates for each quarter are listed below:

  • 1st Oct- 31st Dec
  • 1st Jan- 31st Mar
  • 1st Apr-30th Jun
  • 1st Jul-30th Sept
